A lethal virus that disappeared from Europe twelve years ago has made a comeback. In early December 2025, French health officials confirmed two instances of Middle East Respiratory Syndrome Coronavirus (MERS-CoV) in individuals who had recently traveled from the Arabian Peninsula. The virus is responsible for the death of about one out of every three people it infects—a mortality rate approximately 37 times more severe than that of COVID-19. For healthcare systems still dealing with the aftermath of a worldwide pandemic, this situation serves as both a reminder and a cautionary signal.

Two French patients were quickly isolated, activating contact tracing procedures and increasing awareness throughout European healthcare systems. These cases mark the first confirmed MERS infections in France since 2013, signaling an end to over a decade without the virus on the continent. However, beneath the surface of this apparent stability is an unsettling truth: this virus has never truly left the world. It has remained confined to its endemic areas, primarily Saudi Arabia, waiting for the perfect moment to spread across borders once more.

Understanding the Threat Profile

MERS-CoV is part of the coronavirus family, yet it exhibits significant differences from the virus that was prevalent over the last two years. While COVID-19 had an approximate fatality rate of 1 percent, MERS results in the death of about 37 percent of confirmed cases. This places it among some of the most dangerous pathogens in human history. Since its emergence in 2012, MERS has caused nearly 1,000 deaths across 27 countries. As of December 21, 2025, the year had documented 19 confirmed cases and four fatalities worldwide—figures that seem low until considering the high mortality rate per case. The World Health Organization categorizes MERS as one of the most severe coronaviruses known to science.

Hospital Spread and Susceptible Contact

MERS does not easily transmit through casual interactions in public areas. Rather, it thrives in hospitals, turning these places into environments that amplify its spread. The virus takes advantage of the close moments during medical care: a nurse getting near to help a patient in distress, a doctor inserting a breathing tube while aerosol particles fill the air, and healthcare professionals conducting procedures that produce respiratory droplets. These instances of personal interaction inside hospitals create ideal conditions for quick transmission. Historically, a large proportion of secondary infections have occurred among hospital workers, who then bring the virus back to their families.

This mode of transmission can lead to a diagnostic challenge. A person comes into the emergency room experiencing fever, cough, and difficulty breathing. If there is no clear travel history or specific concern, these common symptoms might result in a COVID-19 test being conducted. Should the result be negative, the individual leaves without additional testing. Days go by. More people are exposed. By the time MERS is identified, chances for stopping the spread have become limited. The virus's incubation period—usually five days but varying between two and fifteen—creates an extended hidden period of risk, during which infected individuals may fly on airplanes, hug loved ones, and go to medical facilities.

The Origin of Zoonotic Diseases and Global Transmission

Each MERS outbreak originates from one source: dromedary camels found in the Arabian Peninsula. A herder comes into contact with an infected animal, allowing the virus to move from animals to humans. Saudi Arabia carries the greatest impact, representing approximately 84 percent of all worldwide cases—2,224 infections and 868 fatalities concentrated in an area where the virus has taken root. However, borders provide no defense. Pilgrims visiting religious locations, business individuals, and families traveling across the globe create pathways for the spread of the disease across continents.

The 2015 outbreak in South Korea highlighted this risk. A single individual who had traveled from the Arabian Peninsula triggered an outbreak that affected 186 people and resulted in 36 deaths. Hospital units were shut down. Medical staff became sick. This chain reaction started with one person on a single flight. France's two cases are the most recent example of a recurring trend that has occurred for more than ten years.

The Lack of Medical Countermeasures

Even after twelve years of being known, a fatality rate above 37 percent, and almost 1,000 deaths, there is still no vaccine. No particular antiviral therapy has been authorized. Contemporary medicine provides only supportive measures—oxygen, artificial breathing, and organ assistance—the same treatments that were available when MERS was first identified in 2012. During the COVID-19 outbreak, the creation of vaccines occurred at an extraordinary pace. However, for MERS, despite its greater deadliness, advancement has come to a standstill.

The World Health Organization issued a warning on December 24, 2025, stating that MERS "still presents a risk in nations where it is found in dromedary camels, with frequent spread to humans." The two cases in France are not unusual but rather an expected outcome in a globalized world where countless people cross borders each day, bringing their possessions and viruses along. Early detection and quick isolation continue to be the main methods available to stop imported cases from causing further spread. The key issue for public health officials in the coming months is whether European healthcare systems remain sufficiently aware of this rare but deadly threat.
