Recent research has revealed a concerning trend: the number of concussion injuries in martial arts that require emergency care has been increasing sharply since the peak of the COVID-19 pandemic in 2020. This rise is particularly notable in practice sessions and classes rather than formal competitions, according to a study conducted by the University of Florida Health.

The findings suggest that while the increase may be linked to more people returning to martial arts after the pandemic, it also points to a need for greater awareness and caution among participants and coaches. Heather Vincent, Ph.D., a senior author of the study and director of the UF Health Sports Performance Center, emphasized the importance of maintaining vigilance even during non-competitive settings.

Focus on Younger Athletes

The majority of concussions observed in the study were among younger boys and teenagers. However, the data also showed an increase in concussions among girls, indicating that the issue affects both genders. Vincent noted that the sharp rise in injuries after 2020 could be attributed to a combination of factors, including rusty techniques and overexuberance as athletes returned to training.

She speculated that the return to sports facilities might have led to less structured environments, where safety protocols may not be as strictly enforced. "Maybe participants are less likely to wear protective gear during practice or training," she said. "There might be less supervision or enforcement of safety measures, especially with children."

The Role of Practice in Injury Risk

Most concussions occurred during practice sessions at recreational and sports facilities. This highlights the need for increased attention to safety during these times, even though the stakes are lower compared to competition. Vincent stressed that every head strike or contact can have serious consequences.

The study also pointed out a gap in existing research: there is no comprehensive assessment of whether coaches and participants, including junior ranks, are adequately trained to recognize concussion symptoms. "It's incredible that this issue hasn't been studied systematically," Vincent said. "We don't know for sure if they're addressing it in small ways, but we need more data."

Data from Emergency Rooms

To gather their findings, researchers analyzed emergency room visits at 100 hospitals nationwide using data from the National Electronic Injury Surveillance System. The study covered a ten-year period from January 1, 2013, to December 31, 2022.

They reviewed 581 detailed cases of concussions related to martial arts and estimated that more than 19,000 cases occurred nationwide during that time. Nearly 71% of the injuries involved male athletes, with children making up 54% of all concussions.

Karate and a broad category labeled "other forms" — which included styles like Krav Maga and capoeira — had the highest number of concussions. Males experienced more concussions than females in mixed martial arts, jiujitsu, and taekwondo. However, in judo and taekwondo, females were more likely to be injured during competition than in practice, likely due to lower participation rates among women in those disciplines.

Where Injuries Occur

Nearly 86% of concussions happened in sports or recreational facilities, with a smaller percentage occurring at schools or homes. Most patients were treated and released, but some required hospitalization, especially those practicing judo, kickboxing, or mixed martial arts.

Vincent supports the idea of a "concussion passport" for athletes, which would track head injuries over time. She explained that repeated concussions can lead to cumulative damage, increasing the risk of long-term brain injury.

"The record follows athletes as they progress through different levels of experience," she said. "It helps coaches understand a participant’s history and ensures appropriate training and medical support when needed."
